Jacksonville Location
Our Jacksonville facility is located at 3444 Southside Blvd., Suite, 103, (904.646.1287) off the service road (next to Tulsa Welding School) on the west side of Southside Boulevard near the intersection with Beach Boulevard.
Traveling on Southside: If you are traveling on Southside Blvd., you will turn at Cargal stoplight, traveling North it’s a left, traveling South it’s a right and then an immediate right onto the service road. We are the third building on the service road north, just past Tulsa Welding School.
Traveling on Beach: If you are traveling West on Beach, go under the Southside overpass and immediately turn left and stay in the right lane. If you are traveling East on Beach, take a right at the intersection and stay in the right lane. Watch for gray block building with a metal green roof and the tower has a profile of the above dog and cat.
Traveling on I-95: If you are traveling on I-95 South, exit onto 295 East Beltway formally 9A (exit 362A) and then exit the Southside Connector (approximately 10 miles) and then follow above directions (approximately 6 miles to Cargal Street). If you are traveling I-95 North, you can take the Southside Blvd (exit 340-North) approximately 8 miles to our clinic or continue to J. T. Butler Blvd (exit 344) and travel East to Southside Blvd (North) exit and follow above directions to the clinic.

Directions From I-295/I-95:
Merge onto I-295 N toward Orange Park. Take the US-17 S/Park Ave. exit, (Exit 10). Turn Left onto US-17 S/Roosevelt Blvd./FL-15 S/Park Ave. Turn Right onto Eldridge Ave. Turn Right onto Wells Rd. Turn Left onto Corporate Way. Entrance is on the Left.
